The Southborough, MA Fire Department received a call for a MVA on route 495 in the south bound lane just north of route 9.
While inroute to the scene the crew on the ambulance was updated that there were three people trapped with a total of four patients.
A-28 requested Box 16 for manpower and the rescue company with the jaws.
On scene A-28 and A-29 found a two car MVA, one SUV rollover and a small pickup truck head-on into a large rock face.
Car 20 (Fire Chief John Mauro) arrived on scene just affter the ambulance assuming command of the scene.
He quickly requested two Life-Flight helicopters, two more ambulances, a rescue from Westborough Fire, and a rescue from Marlborough Fire.
It took 10 minutes to access the driver of the pickup truck who was then pronounced dead at the scene.
His wife and granddaughter were both taken by helicopter in critical condition to U-Mass Medical Center in Worcester.
The driver of the SUV was freed by the Westborough Fire Department and transported als by ground to U-Mass.
